Portland, Oregon – Providence Elder at Home is seeking board-certified/board-eligible Geriatric Medicine nurse practitioner or physician assistant with to join its innovative, comprehensive care and social services program. • Ideal candidate will have 2 years post-graduate clinical experience working with medically complex and/or geriatric patient population.
• Our care team provides the highest level of home-base care with access to full complement of services, including clinical pharmacy, care management, home health, and hospice and palliative care.
• Collaborative, team-based approach working in conjunction with social worker and nurse care manager.
• Autonomy of practice with elbow support from seasoned physicians.
Providence Elder at Home is an innovative care model designed to meet the needs of elders who have specific conditions, diagnoses or symptoms that are not well served by typical medical care models. Elder at Home core teams will draw upon Providence's integrated delivery system to maximize patient function and quality of life, and tailor care to meet the unique needs of the patient, their family and caregivers—24 hours a day, 365 days a year.
